چکیده

Physiological pacing has been a term that has been used to describe many phenomena. It was used over 30 years ago to describe the advent of AV synchronous or dual chamber pacing, later the addition of the rate-responsive mode to pacing, and even more recently to describe biventricular pacing. In the last several years, there has been a resurgence of interest in pacing the conduction system: selective His-bundle pacing and non-selective His-bundle pacing. Despite the widespread use of this term, I believe “selective His-bundle pacing” is the true embodiment of physiologic pacing. In order to better understand the phenomenon, the Journal of Electrocardiology has produced a truly scholarly review of this field. Papers included in this seminar will review the anatomy and pathology of the conduction system, the mechanics and phenomenology of pacing the conduction system as well as the basic science behind understanding the phenomenon of pacing the conduction system. This symposium covers this subject material in a comprehensive

Pacing the Human Cardiac Conduction System

Specialized fiber electrograms were recorded from selected sites along the His bundle and bundle branches during open-heart surgery in 26 patients. The heart was then paced from the same recording sites. Stimuli applied to the proximal His bundle recording site always resulted in His bundle pacing, which was characterized by a stimulus artifact-to-QRS interval which equaled or very nearly equal...

Determination of His-Purkinje refractoriness in man with His bundle pacing.

Consistent His bundle pacing was accomplished in one of 60 consecutive patients. Intracardiac electrophysiologic studies in this patient revealed rapid atrioventricular nodal conduction (AH = 55 msec). While the relative refractory period of the His-Purkinje system as a whole determined by His bundle pacing (using the extrastimulus technique) was identical to that determined by atrial pacing, c...

His Bundle Pacing Or Biventricular Pacing For Cardiac Resynchronization Therapy In Heart Failure: Discovering New Methods For An Old Problem.

Heart failure (HF) is one of the biggest epidemics of modern cardiovascular medicine. Cardiac resynchronization therapy (CRT) with biventricular (BiV) pacing has proven to have an integral role in the management of patients with reduced left ventricular (LV) function and left bundle branch blocks (LBBB). However, CRT with BiV pacing is not always feasible and even when it is, the percentage of ...

Intermittent loss of capture in a His bundle pacemaker: What is the cause?

Case report A 34-year-old man with a history of congenital complete heart block who underwent dual-chamber pacemaker implant at age 6 and multiple generator changes was referred for device replacement and lead extraction owing to malfunctioning atrial and ventricular leads. He underwent lead extraction of a dual-chamber pacing system with subsequent reimplantation of a Medtronic Viva CRT pacema...
